Essential forms for portfolio
Hello Darren,
Other than the CAS forms for each unit and the 10 evidence pieces what other forms need to be uploaded into our dropboxes?
could we have a comprehensive list of the essential paper works needed?
Thank you!

Hi all,
I will be doing an additional ad-hoc revision session next week on Tuesday 14/01/25 for those who will be sitting the A&P part 1 resit paper on 16/01/25 (14:00hrs) - the revision session will start at 10am via zoom (usual zoom password). This session will not be recorded.
Kind regards,
Darren
Dear cohort,
I would like to inform you that due to an administrative error there has been a delay in sending your A&P part 2 exam papers (sat on the 14th November) to the RSPH. What this mean in practice is that your exam results will be delayed by a further 4 weeks than what you was expecting – so estimated around the end of January 2025.
I deeply apologise for the delay and appreciate that it must be frustrating for you all. I understand the importance of timely communication and will update you ASAP when your results are back.
Regards,
Darren
Hi all,
Now that you have completed your theory training with me and will hopefully be working on your portfolios - please do not forget the following:
Once you have uploaded all your portfolio work your mortuary can then formally apply for the EPA (use the 'links' tab found on this website). The EPA application page will open for the whole of February 2025.
To upload your work you will need access to your own individual drobox link - email me directly when you are ready to upload - do not leave it too late! You may add, delete and update evidence work within your e-portfolio at any time up until the end of 28th February 2025.
I have updated the past examples 1 with some recommendations on how to improve these evidence pieces as discussed on the last session:
Please note: You are only eligible for EPA once all exams and assignments have been passed!
Hi, apart from the CAS form for indexing your evidence work, you will need to use the 3 different template types for your evidence wok for each unit:
3 x reflections
3 x witness statements
4 x other work based objective
The only other mandatory form is the 'RSPH candidate evidence portfolio front sheet' - this is a declaration form to say the portfolio is your own work - a blank copy of this is in everyone's e-portfolio.
The following are not mandatory but may be included:
Signature sheet template - only use if a specific staff member has been named in your portfolio for some reason (other than evidence signatures).
Mentor progress review meeting template - again only if used and completed.
The CPD log is not required for EPA but may be asked for if you apply for professional registration with the science council after the course (needed to get onto the level 4 diploma).
That's all!
P.S Remember to ask for access to your e-portfolio if you are going to apply for the EPA in February - you should only apply for EPA (via 'Links' on the training website) after you have uploaded all your evidence pieces.
