Work Based (Objective) Evidence examples
Management of the deceased

Example 1: Receipt and release SOP supplemented by local mortuary release form (to FD). Note that the candidate has included only one page taken form the SOP (i.e. not the whole SOP) to explain the main contents of the document. Also included are text boxes explaining with arrows pointing the the key areas being described. This evidence piece could be used to cover a number of LO for this unit.

Example 2: Local forms used to record tissues / samples within the department - this evidence piece show good audit protocols for the identification and control of tissue kept within the department as well as documentation of their eventual disposal / repatriation. When using forms as an evidence piece it it good practice to briefly explain the various elements of the document to the assessor (who may not be familiar with local procedures).

Example 3: Initial ID and receipting documentation of the deceased - again this strong evidence piece contains a number of objective photos / images taken form the candidates work place (note signature block not included - although available on original). As this is a level 4 portfolio evidence piece the candidate has included part of a 'process flowchart' or pathway used to help assist with receipting of patients.

Example 4: Tissue labelling and handling - safe storage and unambiguous labelling and identification of all tissues held by the department will ensure that legal and regulatory requirements for the storage and release of tissues are complied with. This candidate has included numerous images for the various key aspects pertaining to this area (e.g. images pertaining to hazards, COSHH, storage, audit trails, handling and labelling of tissue).

Example 5: SOP for receipting of the deceased supplemented by copy of mortuary register - being a level 4 evidence piece the candidate has further explained and differentiated ID check protocols for both hospital and community based deaths. This evidence piece covers similar LO for this unit as example 3 above. Again notice how the candidate included only relevant sections from an SOP and also explains the key elements of the register book that must be completed.

Example 6: Receipt and release of fetus - this candidate has explain procedure (s) relating specifically to fetuses and their release following PM. If the candidate had explained further the procedures relating to the PM then this piece of evidence may have instead been used for unit 4.9 (specialist PM requirements). Note that once an evidence piece has been used for a particular unit it cannot be used again for anther different unit.

Candidate has used a colour code to key to explain the different element of the excel register sheet and has also anonymised any patient details. A local release form has also been included to supplement the evidence piece.
