Work Based (Objective) Evidence examples
Deliver and evaluate training in APT

Example 1: Competency staff log - here the candidate has designed a new competency for support staff working within the mortuary that includes both trainee and trainer sign of as well as providing an opportunity for feedback for any of the tasks. Candidate has annotated work to explain the various areas of the document.

Example 2: Training activitycandidate has provided evidence of training given to a trainee staff member and is working in accordance to local SOP. Note that there are two objective images contained in this evidence piece:

  • Scan of an SOP (relevant procedure section)

  • Photo of training given

Example 3: Training plan - the candidate has used this training plan to help manage dates and schedules of training activities and tasks (competencies) requiring sign off for a member of mortuary support staff. Training plan sheet has been annotated to provide further explanation.

Example 4: Training feedback form - as this unit for the level 4 diploma requires evidence that training given has been evaluated this evidence piece could be linked to cover these learning outcomes. This evidence piece could be further supplemented by  inclusion of feedback answers (or summary of responses over a given time).

Example 5: Training [competency] booklet - this evidence piece shows the scope of practice training required for new trainees and discusses the essential documentation contained within the training pack (e.g. timetable, competencies requiring sign off and progress reports). Candidate has broken down the booklet into some of the key areas and briefly described each and also annotated the images. This evidence piece could be linked to all the learning outcomes for this unit.

Example 6: Mock exams for trainee staff - This evidence piece demonstrates consolidation of training given by providing evidence relating to mock exams given to trainee staff. The candidate further explains the scope and type of questions contained within the exam (with example question) and how feedback is given to the trainee after the exam has been marked.

Example 7: Presentation given to other mortuary staff - here the candidate has given some context to why and how the presentation was given and then included a screenshot of the front page of the PowerPoint presentation that contains key information such as candidates name, date and title (whole presentations are not required in evidence pieces).

Example 8: Mentor progress review meetings - here the candidate has provided evidence of documented progress review meetings that they have with their trainee staff. Candidate has included anonymised examples of progress meeting reports with further explanations included. This evidence piece could be linked to all the learning outcomes for this unit.
