Supervision of mortuary functions
Example 1: Works carried out to increase mortuary storage space. Good use images showing local form in use (to request work to be carried out) and a photo of the new mobile storage fridge units after completed of works. Candidate has annotated work with text boxes to explain images (text within these boxes may also be hand written to further personalise work).
Example 2: Huddle meeting form - this shows the agenda from a local meeting detailing some of the roles of the candidate during the meeting. This evidence piece may be related to many of the L.O for this unit (e.g. quality, supervision, management , maintaining mortuary services etc.). Candidate has included a copy of the agenda and annotated it to explain the format of the meeting.
Example 3: Fridge temperature and space management - this evidence piece shows various objective images relating to records for temperature control and occupancy fridge space management for the deceased. The candidate has also explained how this data could be used for discussions at local performance meetings and also used for statistics to external agencies. Candidate has annotated work with text boxes to explain the images (text within these boxes may also be hand written to further personalise work).
Example 4: CAPA raised from a local audit - this evidence piece shows a CAPA (Corrective Action / Preventative action) log being raised for a non-compliance identified form a local audit carried out in the department. As well as an annotated screen shot of the CAPA the candidate has further explained the non-compliance (failure to keep up to date LEV records) by showing an image of the record sheet that was not updated.
Example 5: Discard of tissue processes - this evidence piece shows an SOP, communication email and electronic record explaining the audit trail for local tissue disposal. Candidate has used a series of images to better explain the sequential steps in the process. Candidate has also anonymised any staff and patient details by using a black marker pen before scanning their work to their portfolio.
Example 6: Stock and consumables management - this evidence piece shows a local form in use for routine stock checks - this has been supplemented with a screenshot of a local SOP on how to order and receipt new consumables. Note that the candidate has only included a screen capture of the relevant section from within the SOP (and not the whole document).
