I can only assume that someone asked the HTA during last weeks session about an example viewing form. Please see form below:
The form is fairly basic and was designed to be used in the following way:
The family complete the form on arrival to the mortuary for a viewing.
The details are then checked against the ID Bands on the body and details given at the time of the viewing booking, immediately prior to the family being escorted into the viewing room to see their loved one.
Other additions to the form could include an area for the APT undertaking the final ID check to sign.
If ID bands do not have an address on them, a date of death could be used- it is vital 3 forms of ID provided by the family can be checked against information held on the body. Most family members will not know the hospital number or NHS number of their loved one.
